A peel or chemical peel is the process of applying a mild acid to the skin which injures the upper layers of the skin to cause the deeper layers of the skin to produce new fresh skin. This causes the skin to tighten; eliminating fine to moderate wrinkles. This has the additional benefit of causing the upper layer of the skin to slough off, removing superficial pigmented spots (‘sun spots’) and pre-cancer lesions such as Actinic Keratosis. There is skin flaking on days 2-4. It takes 6-12 weeks to see the full skin tightening effect (depending on depth of the peel).

Chemical peel medium depth

Actinic Keratosis is a pre-cancer that lives in the superficial layers of the skin. It is similar to a real cancer (like squamous cell carcinoma), however actinic keratosis lives in the temporary layer of the skin (the top layer or epidermis). If it is allowed to stay and grow, it will grow deeper and deeper, eventually it will grow into the dermis (the permanent layer of the skin). Once it is in the dermis it is then a true squamous cell carcinoma and would need to be cut out for cure from that skin cancer. When a person has actinic keratosis, is sign of broad skin damage (years of being in the sun without protection).

 

There are various skin treatments which can help to prevent the progression of actinic keratosis into squamous cell carcinoma. Whatever treatment is chosen, it must destroy the actinic keratosis. This can be done many ways.  If it is one spot on the face, electrocautery can be performed to destroy the spot. Liquid nitrogen can destroy the spot however there is risk of eye injury and it destroys a large area in an uncontrollable manner which can result in bad scarring. Efudex (fluorouracil topical) is a cream that can be applied to the actinic keratosis spots for 6 weeks twice a day. The problem with this treatment is ‘raw-open wound look’ of the spots makes it difficult for most patients to go to work or be out in public. A laser is very exact but often expensive. We prefer a chemical peels for several reasons. For the face, chemical peels are very safe, can be controlled in a very exact way and treat the entire face, not just one spot.  Chemical peels can be tailored to your lifestyle, the post peel recovery can be very short and mild or even have almost no social downtime.
